When sending a sample to Plastic Failure Labs, please download the Sample Submission Questionnaire.  Please fill out the questionnaire as appropriate.  The completed questionnaire can be be emailed to plasticfailure@gmail.com  or printed off and sent along with the samples for failure analysis.
